SUMMARY

The following characteristics should be noted:
1. Site
2. Size
3. Stage
4. Edge: sloping (venous), punched out (arterial), heaped (malignancy)
5. Wound bed: presence of necrosis, underlying structures visible, presence of sinus tracts
6. Surrounding tissue: erythema, induration, edema, cellulitis
7. Exudate: odor, color, volume
8. Surrounding lymphadenopathy
9. Neuro-vascular status
