SUMMARY

1. Most severe nerve injury.

2. Complete disruption of axons, myelin sheath, endoneurium, perineurium, & epineurium.

3. Wallerian degeneration occurs.

4. Recovery requires operative repair and the prognosis is variable, ranging from complete recovery to no recovery.

5. Factors that influence the degree of recovery: age of the patient, type & severity of injury level of injury (more proximal injuries confer worse prognosis), associated injuries, delay in regeneration leads to shrinkage of endoneural sheaths.
