1. Wound/dressing review and management
2. Investigation of falls, falls risk factor modification, osteoporosis management and other outstanding medical issues
3. Pain management: rationalize analgesia
4. DVT prophylaxis: in conjunction with Orthopedic surgeon
5. Hip precautions
6. Equipment review: chair, toilet, mobility aids
7. Blood investigation review: electrolytes, inflammatory markers, calcium, Vitamin D and B12, folate, bone metabolic markers
8. PT: ROM exercises (within limits of hip precautions), mobility, balance, transfer practice

10. Social work review: services, finances
11. Discharge planning: home/respite/placement, services, caregivers, equipment, transportation, transfers
