HIP FRACTURES-DVT

SUMMARY

1. Recommendation is for chemical DVT prophylaxis (chemical better than mechanical).

2. The American College of Chest Physicians (ACCP) has estimated that the baseline risk for 35 days beyond surgery is 4.3%, with the highest risk occurring in the first 7 to 14 days (1.8% for symptomatic DVT and 1% for PE); rates fall in the subsequent 15 to 35 days (1 and 0.5% for symptomatic DVT and PE, respectively).

3. Chemical prophylaxis: Clexane 40mg daily, Heparin 5000mg BD, Rivaroxaban 10mg daily, Apixaban 2.5mg BD.

4. Duration of chemical prophylaxis: 35 days.

5. 30-80% chance of developing DVT in major orthopaedic surgery if prophylaxis is not commenced.
