SUMMARY

1. Synonym: reflex sympathetic dystrophy (RSD), shoulder-hand syndrome, or Sudeck atrophy.

2. Syndrome of variable course and unknown etiology characterized by pain that is disproportinate to the inciting event, swelling, and vasomotor dysfunction of an extremity.

3. Clinical: most consistent early diagnostic signs are shoulder pain with flexion, adduction and external rotation; no pain in the elbows; pain on wrist dorsiflexion with edema.

4. Shoulder and hand pain preceded by rapid ROM loss & pain disproportionate to signs.

5. Other signs: changes in skin texture on the affected area (may appear shiny and thin); abnormal sweating, nail growth patterns.

6. Reported in 12-25% of hemiplegic stroke patients (as shoulder-hand syndrome).
