SUMMARY
PERSON(S) RESPONSIBLE:
1. Guardian: if person is under guardianship. Areas of guardianship (accomodation, health care, medical & dental consent) must be specified.
2. Enduring guardian: if the patient has appointed one.
3. Spouse: can be defacto or same sex partner, there must be close continuing relationship, ex partners are not allowed.
4. Primary caregiver: determined if they have provided, or have arranged to have provided domestic services and support other than for renumeration.
5. Close relative or friend: as long as they do not receive remuneration.
