SUMMARY

1. Methadone is a potent NMDA receptor antagonist, NMDA activation has been implicated in neural plasticity (central sensitization) & as a mediator of opioid tolerance.

2. Methadone accumulates in tissues with repeated dosing, creating an extensive reservoir, and is highly protein-bound.

3. Half-life varies from 7 hours to 5 days.

4. Dosing changes must be done at least every 5 to 6 days.
