TBI-ISSUES REGARDING GATE LEAVE

SUMMARY

The following must be considered prior to allowing gate leave:
1. Wheelchair: obtain equipment, ensure patient's home/place is wheelchair accessible
2. Equipment: such as Pelican belt for the patient
3. Transport: wheelchair taxi to be organized
4. Chair to car transfers and vice versa
5. If wheelchair taxi not available, patient and relative to be thought car transfers
6. Supervision from relatives or partner for toileting and transfers, pushing the chair
7. Organize medications for the patient to take home and organize supervision of consumption of medications. Teach patient/family administration of medications (such as insulin, Clexane injections)
8. If incontinent, organize continence aids
9. Education of patient: regarding falls, signs and symptoms of recurrent stroke/seizures and important to avoid alcohol and cigarette smoking
