SUMMARY

1. Grade 1: non-blanchable erythema.

2. Grade 2: partial thickness loss of dermis, shallow ulcer.

3. Grade 3: full thickness loss of dermis, exposure of subcutaneous tissue.

4. Grade 4: full thickness loss of dermis, exposure of bone, tendon, and muscle.

5. Unable to stage: full thickness loss of dermis, actual depth is obscured from slough and/or eschar.
