SUMMARY

1. Same principles as for physical modalities: know what and why you are referring.

2. Know the provider: are they equipped to provide neuropsychological assessment and CBT?

3. Include tools to evaluate and monitor patient's progress:
- Tampa scale: to assess fear of pain
- PSEQ (Pain Self-Efficacy Questionnaire): to assess self-efficacy for pain management
- DASS-21: to assess for depression, anxiety and stress
- ODI (Oswestry Disability Index): to assess for overall disability

4. Re-evaluate progress regularly, define goals and manage expectations.

5. Multi-disciplinary meetings to discuss patients are useful in aligning goals between the various therapists and clinicians.
