SUMMARY

1. Use a multimodal approach, combining non-pharmacological with pharmacological therapies.

2. Patient education. Negotiated goals.

3. Aim to get pain to a tolerable level to optimise function and emotion, not to totally eradicate pain.

4. Pharmacotherapy for chronic pain has at best moderate efficacy, but often a high placebo response.

5. Reassess response to treatment. Consider a trial off a medication. It can be recommended if pain exacerbates.

6. Opioid therapy: Time limit for acute pain. Dose limit for chronic pain.
