SUMMARY

TIMING
1. Time of onset uncertain or > 4.5 hours; > 9 hours with perfusion mismatch

PRESENTATION
2. Poor prognosis: coma, fixed, dilated pupils, dense hemiplegia (indicative of large area of infarction)
3. Advanced liver or heart disease

RISK OF HAEMORRHAGE
4. ICH present
5. SAH, septic emboli present
6. BP > 180/110mmHg
7. Patient on anticoagulants, received Heparin within 48 hours, hx of coagulopathy, INR > 1.4
8. Thrombocytopenia
