3. M - metabolic; DM, acromegaly, hypothyroidism

4. I - infection; e.g. HIV, GBS, lyme disease
5. T - tumor; e.g. lung CA

6. B - B12, B1, B5 & B6 deficiency
7. I - idiopathic
8. C - CTD or vasculitis; e.g. SLE, PAN
9. H - hereditary
