PULMONARY EMBOLISM-ANTICOAGULANT CONTRAINDICATIONS

SUMMARY

1. Uncorrected major bleeding disorder (thrombocytopenia, hemophilias, liver failure, renal failure)

2. Uncontrolled severe hypertension (systolic > 200 mmHg, diastolic > 140 mmHg)

3. Potential bleeding lesions (active peptic ulcer, esophageal varices, ruptured aneurysm, recent trauma or surgery to head/orbit/spine, recent stroke, intracranial or intraspinal bleed)

4. Repeated falls or unstable gait

5. LMWH is contraindicated in renal clearance < 30 cc/min

6. Concurrent NSAID use: increases risk of GI bleeding; if not contraindicated, stop NSAIDs
