SUMMARY

Predictors of failure:
1. Acidosis ( pH < 7.25)
2. Marked new onset hypoxia (PaO2/FiO2 < 200 mmHg)
3. Respiratory distress signs (RR > 25/min)
4. Non-pulmonary organ failure, particularly cardiovascular dysfunction
