SUMMARY

1. Lesion of the underlying subchondral bone with/without involvement of the overlying cartilage. Commonly involve the medial femoral condyle.

2. Grade 1: compression of subchondral trabeculae with preservation of the cartilage.

3. Grade 2: incomplete detachment of an osteochondral fragment.

4. Grade 3: complete avulsion of an osteochondral fragment without dislocation.

5. Grade 4: complete avulsion of an osteochondral fragment with dislocation (loose body).

6. Surgical techniques can be palliative (arthroscopic debridement), reparative (microfracture), or restorative (autologous chondrocyte implantation, osteochondral graft).
