SEVERE ATTACK
1. Unable to complete sentences
2. Respiratory rate > 25/min
3. Pulse rate > 110 beats/min
4. Peak expiratory flow 33–50% of predicted or best
LIFE-THREATENING ATTACK
1. Peak expiratory flow < 33% of predicted or best
2. Silent chest, cyanosis, feeble respiratory effort
3. Bradycardia or hypotension
4. Exhaustion, confusion, or coma
5. ABG: PaCO2 > 32mmHg, PaO2 < 60mmHg, pH < 7.35
